This is the Montana version of the W-4 where you choose your state tax withholding preferences.
This form confirms you’re legally allowed to work in the U.S. — you’ll need to provide ID documents for it.
Use this to tell us where to deposit your paycheck — no paper checks needed.
This gathers your contact details, emergency contact, and other basic info we need to set you up in our system.
